The form is in PDF format and can be printed on your computer or device, saved to your records and submitted via your family’s rep or via the committee email address

Complete the form as best as you can. A sample copy is on the next page.  However, any way you choose to submit is fine, we don’t want you downloading and installing things you won’t otherwise use. Please include stepchildren as well.
